Conclusion: Heterogeneity across studies makes it difficult to conclude any benefit or detriment to consuming ketone supplements on physical performance. This systematic review discusses factors within individual studies that may contribute to discordant outcomes across investigations to elucidate if there is sufficient evidence to warrant recommendation of consuming exogenous ketone supplements to enhance physical performance.

The short answer. There isn't much data on Ketone Esters yet — 4 mentions, 1 with a clear outcome — so treat these numbers as anecdotal. There is a moderate body of research — 6 meta-analyses and 5 trials.
Out of 4 mentions; the other 75% didn't say how it went and aren't counted.

What the research says
Systematic reviews and meta-analyses from PubMed, with their published conclusions.
At least one meta-analysis, or 5+ randomized controlled trials. The grade measures how much human research exists — read the conclusions below for what it found.
Conclusion: Our results suggested that ketone therapy significantly improved the cardiac systolic function and structure in rodents with HF, and had no effect in rodents non-HF. Thus, ketone intervention may be a promising treatment for HF patients.
PubMed 40252394 ↗Conclusion: Exogenous ketosis shows potential in neurological, metabolic, and cardiovascular disorders, while evidence in psychiatric and inflammatory conditions remains scarce and preliminary. Ketone esters appear preferable for effective and tolerable ketosis. Future research should focus on identifying responsive patient populations, optimizing treatment regimens, and conducting long-term clinical trials with hard endpoints to validate these findings.
PubMed 41097203 ↗Conclusion: Limitations of the literature to date include the range of ketogenic interventions currently assessed in the literature (i.e., primarily diet or medium-chain triglyceride interventions), with fewer studies using more potent formulations (e.g., exogenous ketone esters). Collectively, the strongest evidence to date exists for cognitive improvement in individuals with mild cognitive impairment and in individuals with mild-to-moderate Alzheimer's disease negative for the apolipoprotein ε4 allele. Larger-scale, pivotal trials are justified in these populations. Further research is required to optim
